Assessment for advancing community transformation

The Assessment for Advancing Community Transformation (AACT) tool is a research-based assessment to help coalitions and groups gain more insights into their collaborative journey. The tool was developed by an experienced team of practitioners and researchers from the University of Wisconsin's County Health Rankings & Roadmaps program, Georgia Health Policy Center, and Institute for Healthcare Improvement's 100 Million Healthier Lives Initiative, with funding from the Robert Wood Johnson Foundation.
